Admit to ICU; monitor for sepsis and raised ICP
Give dexamethasone with first dose of antibiotics
Perform lumbar puncture if no contraindication (ICP, focal deficit, seizure)
Obtain blood cultures before antibiotics
Start empiric IV antibiotics immediately (ceftriaxone + vancomycin ± ampicillin if >50 or immunocompromised)
