LP with CSF HSV PCR; obtain MRI brain and EEG for temporal spikes/periodic discharges
Support airway, treat seizures per SE protocol; manage ICP and temperature
Start IV acyclovir 10 mg/kg q8h immediately on suspicion (renal-dose adjust)
If PCR negative but high suspicion persists, continue acyclovir and repeat LP in 24–48h
Treat 14–21 days; monitor creatinine and ensure adequate hydration
